Plymouth Deep End General Practice is a collaborative network of GP practices, clinicians, educators and community partners working in some of the most socioeconomically deprived communities in Plymouth.

Our mission is simple:

To reduce health inequalities by delivering compassionate, trauma-informed, community-focused healthcare to people facing the greatest barriers to health and wellbeing.

Why We Exist

Health inequalities in England are wide, persistent and growing.

People living in the most deprived communities experience:

• Shorter life expectancy
• Earlier onset of chronic illness
• Higher levels of mental health difficulties
• Greater exposure to trauma and adverse childhood experiences
• Reduced access to consistent healthcare support

General practice in these communities faces additional pressure:

• Higher patient complexity
• Increased workload
• Workforce shortages
• Greater demand with fewer resources

Plymouth Deep End was created to respond directly to these challenges.

What We Do

Deliver Frontline Care

We support GP practices working in areas of high deprivation across Plymouth, helping provide accessible, relationship-based healthcare for patients with complex physical, psychological and social needs.

Our practices work with people experiencing:

• Multiple long-term conditions
• Homelessness
• Addiction and substance misuse
• Trauma-related illness
• Mental health difficulties
• Social exclusion and poverty

Build Trauma-informed Services

Many patients living with long-term illness have histories of trauma, instability or adverse childhood experiences.

We aim to provide care that is:

• Compassionate
• Non-judgemental
• Relational
• Safe and consistent
• Aware of the impact of trauma on physical and mental health

Our Deep End trauma-informed clinics focus on patients with:

• High levels of chronic illness
• Frequent GP or emergency department attendance
• Complex mental and physical health needs
• Long-term pain or dependence on opioids and other medications

 
Support Vulnerable Communities

We work beyond the walls of traditional healthcare.

Our outreach and partnership work includes:

• Homeless outreach clinics
• Addiction and recovery services
• Hepatitis and liver health screening
• Social prescribing
• Community wellbeing events
• Advocacy for underserved populations

We collaborate closely with charities, voluntary organisations, public health teams, community groups and local services.

Educate Future Health Professionals

Education is central to the Deep End approach.

We provide placements and teaching for:

• Medical students
• GP trainees
• Nursing students
• Occupational therapy students
• Clinical psychology trainees

Our aim is to inspire future clinicians to understand health inequalities and build careers serving communities with the greatest need.

Advocate for Fairer Healthcare

We believe healthcare systems should reflect need.

Plymouth Deep End advocates for:

• Fairer GP funding
• Better workforce support
• More equitable access to care
• Stronger community services
• Policies that address the social causes of poor health

We work to highlight the reality of the “Inverse Care Law”, where communities with the greatest health need often have the least healthcare resource.

Research and Innovation

We contribute to research and service development focused on:

• Models of care for complex multimorbidity

By combining frontline experience with research and advocacy, we aim to improve outcomes locally and contribute to wider national change.

Together, these practices support patients living at the “deep end” of health inequality.
